Golden Home Care – Terms and Conditions

1. Introduction
These Terms and Conditions govern the use of services provided by Golden Home Care, a division of DuMonde Services Management Inc. By engaging our services, you acknowledge and agree to these terms. We reserve the right to amend these terms at any time without prior notice.

2. Scope of Services
Golden Home Care provides a range of home-based support services including:
- Senior In-Home Care
- Mental Health Support Services
- Respite Care for Family Caregivers
- Family Caregiver Support
- Palliative and End-of-Life Home Care
All services are customized to meet the unique needs of each client and are subject to a personalized care plan.

3. Service Eligibility
Clients must undergo an initial assessment to determine eligibility and suitability for our services. Golden Home Care reserves the right to refuse or discontinue services if the safety of the client or care provider is at risk.

4. Client Responsibilities
Clients and/or their representatives agree to:
- Provide accurate and complete information relevant to care
- Ensure a safe and respectful environment for caregivers
- Notify us promptly of any changes in health status or household conditions
- Treat all staff members with dignity and respect

5. Payment Terms
Fees for services are discussed and agreed upon before care begins. Invoices are issued on a weekly or monthly basis, depending on the service plan. Payment is due upon receipt unless otherwise specified in writing. Late payments may incur additional charges.

6. Cancellation and Rescheduling
Clients must provide at least 48 hours' notice to cancel or reschedule an appointment. Failure to do so may result in a cancellation fee. Golden Home Care may reschedule services in emergency or unforeseen circumstances.

7. Confidentiality
All personal and health-related information shared with Golden Home Care will be treated as confidential and handled in accordance with the Personal Information Protection Act (PIPA) and other applicable privacy laws.

8. Liability
Golden Home Care and its staff are not liable for any injury, loss, or damage sustained by the client or others unless caused by proven negligence. We are not responsible for medical emergencies or outcomes beyond our scope of service.

9. Termination of Services
Either party may terminate services with written notice. We reserve the right to discontinue services due to non-payment, abuse, or unsafe working conditions.

10. Dispute Resolution
Any disputes arising under these terms will be resolved through mediation or arbitration under the laws of British Columbia, Canada.
